About This Program
New York's Social Care Networks (SCNs) are regional hubs designed to bridge the gap between healthcare and social services. Established under the New York Health Equity Reform (NYHER) 1115 Waiver, these networks connect Medicaid members with critical "Health-Related Social Needs" (HRSN) like housing, food, and transportation.
