What is the Social Care Network?
New York's Social Care Networks (SCNs) are regional hubs designed to bridge the gap between healthcare and social services. Established under the NY Health Equity Reform (NYHER) 1115 Waiver, these networks connect Medicaid members with critical Health-Related Social Needs (HRSN).

Who is Eligible?
Social Care Network services are available to New York Medicaid members who have been identified as having Health-Related Social Needs (HRSN).
- Must be enrolled in New York Medicaid
- Identified as having social needs affecting health
- Referred by healthcare provider or self-referral
